COL-B Business Assessment Privacy Notice

The COL-B Business Assessment collects the contact information, organization profile, and assessment responses you choose to submit. The application uses those responses to generate your assessment report, calculate deterministic scores, identify practical opportunities, and store a completed submission record.

Cognitive Workstead may review submitted assessments to understand the request, validate the highest-value opportunities, respond to a requested consultation, and improve the usefulness of this assessment. Contact information may be used to respond if you schedule a consultation or ask for a copy of your assessment.

Do not submit protected health information, payment-card information, passwords, authentication credentials, government identification numbers, highly sensitive personal information, trade secrets, confidential customer records, or other sensitive data. Treat open-text fields as a place for general operational descriptions only.

This site runs on the ChatGPT Sites platform, which processes requests needed to operate the application, serve pages, store submitted assessment records where configured, and support site administration.
